Boarding Information Consent Form

 For the health of all our boarding friends, all pets must be up-to-date on vaccinations, including Bordetella for dogs (highly recommended for cats, but not required). We also highly recommend Canine Influenza (H3N8) for our grooming dogs. It is a relatively recent virus that can be endemic in social situations such as grooming, boarding, daycare and dog parks. Our feline friends must be current with their Leukemia vaccine or have proof of a negative Leukemia test. If your pet is not on a flea preventative, he/she will be given a Capstar® tablet at no charge to you. Capstar® kills adult fleas. This helps keep our boarding area and your home flea free!
